Dr. Jeffrey D Wasserman, Doctor of Osteopathy
Dr. Wasserman has been practicing medicine for more than 45 years. More than 30 of those years he has been in practice with North Florida Medical Centers.
Mrs. Elaine Larkins, Advanced Practice RN
Mrs. Larkins received her Bacheolor of Science in Nursing in 1980 and her Masters of Science as a Family Nurse Practitioner in 1988. She worked in hospitals on the floor and as OB/GYN Nurse Practitioner. She joined North Florida Medical in 1991 and has been with the group for over 30 years.
Zaporia Smith, FNP-C, Family Medicine
Zaporia Smith is a dedicated Family Nurse Practitioner with a diverse background in dialysis, home health, long-term care, and primary care. Her journey in healthcare began in high school, where she first trained as a CNA and continued advancing through nursing to earn her MSN-FNP. She believes in providing respectful, high-quality care while creating a welcoming and enjoyable experience for every patient. Outside of work, Zaporia enjoys self-care, spending time with family and friends, and traveling.
Patrick Brown, Internal Medicine, MD
Chief Medical Officer
Dr. Brown specializes in Internal Medicine and has over 20 years in the practice of medicine. In addition to holding clinic hours at North Monroe Medical Center, Dr. Brown has served as the Chief Medical Officer for North Florida Medical Centers for the past 14 years.
Dr. Brown was awarded the Jessie Trice Outstanding Leadership Award from the Florida Association of Community Health Centers (FACHC) at their 2022 Annual Meeting.
Jacara Mosley-Lee, MSN, APRN, PMHNP-BC
Psychiatric Mental Health Nurse Practitioner
Jacara Mosley-Lee, MSN, APRN, PMHNP-BC, is a compassionate Psychiatric Mental Health Nurse Practitioner with more than 10 years of experience supporting and advocating for individuals living with behavioral health concerns. She earned her undergraduate nursing degree from Florida State College at Jacksonville and her graduate degree from St. Thomas University in Miami.
A lifelong resident of Hamilton County, Florida, Mosley-Lee has a deep connection to the community she serves. As a proud mother of three sons, her personal experiences have further strengthened her commitment to supporting individuals and families, particularly those in rural and underserved communities.
Mosley-Lee is passionate about providing accessible, patient-centered mental health care and helping reduce the stigma surrounding it. She strives to create a safe, supportive environment where every individual feels heard, respected, and valued while feeling empowered to make their well-being a priority.
Through compassionate care and a patient-centered approach, Mosley-Lee is dedicated to helping each of NFMC’s patients feel supported on their journey toward improved mental health and overall wellness.
