General Practice

Dr. DeLeon earned his Doctor of Medicine degree from De La Salle University College of Medicine in 1996. He was inspired to pursue medicine by his passion for helping others and his desire to make a meaningful, lasting impact on the health and well-being of his community. His care philosophy centers on treating each patient as a whole person. He listens attentively, builds trust, and works in partnership with patients so they feel informed, supported, and empowered in every stage of their care.

Outside of the clinic, Dr. DeLeon enjoys spending time with family, gardening, trying new recipes, and staying engaged in stocks and options trading.

 Family Medicine

Mrs. DeShala Frazier is a Family Nurse Practitioner with many years of experience in Nursing. She is a CPR Instructor for the American Heart Association. Mrs. Frazier is a long time resident of Madison FL and is a wife and Mom. Her interests include being a Praise and Worship Leader and a founder of Take Flight Inspiring Women Ministry in which she mentors young ladies ages 10-17. She enjoys crafting and spending time with family and friends.

Chief Medical Officer

Dr. Brown specializes in Internal Medicine and has over 20 years in the practice of medicine. In addition to holding clinic hours at North Monroe Medical Center, Dr. Brown has served as the Chief Medical Officer for North Florida Medical Centers for the past 14 years.

Dr. Brown was awarded the Jessie Trice Outstanding Leadership Award from the Florida Association of Community Health Centers (FACHC) at their 2022 Annual Meeting. 

Psychiatric Mental Health Nurse Practitioner

Jacara Mosley-Lee, MSN, APRN, PMHNP-BC, is a compassionate Psychiatric Mental Health Nurse Practitioner with more than 10 years of experience supporting and advocating for individuals living with behavioral health concerns. She earned her undergraduate nursing degree from Florida State College at Jacksonville and her graduate degree from St. Thomas University in Miami.

A lifelong resident of Hamilton County, Florida, Mosley-Lee has a deep connection to the community she serves. As a proud mother of three sons, her personal experiences have further strengthened her commitment to supporting individuals and families, particularly those in rural and underserved communities.

Mosley-Lee is passionate about providing accessible, patient-centered mental health care and helping reduce the stigma surrounding it. She strives to create a safe, supportive environment where every individual feels heard, respected, and valued while feeling empowered to make their well-being a priority.

Through compassionate care and a patient-centered approach, Mosley-Lee is dedicated to helping each of NFMC’s patients feel supported on their journey toward improved mental health and overall wellness.
