North Florida Medical Centers (NFMC) is a Federally Qualified Health Center (FQHC). We offer a Sliding Fee Scale Program to help make healthcare more affordable for patients who qualify. This program allows eligible patients to receive care at a reduced cost based on household income and family size.

 

NFMC is partially supported by federal grants, which help us provide services to our communities. However, these grants do not cover the full cost of care. Patient payments—based on the sliding fee scale—are essential to helping us continue offering high-quality, affordable healthcare to those who need it most.

WHAT IS THE SLIDING FEE PROGRAM?

The Sliding Fee Scale is a program that adjusts the cost of services for eligible patients based on their income and household size. Eligibility is determined using Federal Poverty Guidelines, which are updated annually by the federal government.

Depending on eligibility, fees may be reduced to a minimum fee or a percentage of the full charge. Patients with insurance may also benefit from the Sliding Fee Scale for services that are not covered by their insurance plan.

HOW DO I QUALIFY?

o determine eligibility, NFMC is required by federal guidelines to collect certain information from patients, including income and household size. Eligibility decisions cannot be made until all required documentation has been provided.

HOW TO APPLY FOR SLIDING FEE SCALE

This is what you need to bring prior to or on the day of your appointment. In order for you to qualify for our Sliding Fee Scale, you must fill out an application and provide one of the documents below.

  • Pay stubs from the most recent four (4) weeks

  • Most recent federal income tax return

    • Self-employed patients must include Schedule C

    • Tax returns expire on April 15 of the following year

  • Workers’ Compensation documentation (most recent four weeks)

  • Unemployment Compensation documentation (most recent four weeks)

  • Disability or SSI benefits letter (temporary or permanent

  • Social Security income

    • Proof of monthly deposit or annual benefits letter

  • Documentation from a local, state, or federal agency

  • Completed Employment Verification form

    • Required for household members paid in cash

  • Letter of support from a family member

    • Used when none of the above documents are available

  • Statement of no income, if applicable

  • Any other documentation that verifies income or financial status

Apply for our sliding fee program. Our current slide fee program starts as low as $25 for a visit & includes most labs.

As a Federally Qualifying Health Center, we may be able to discount your deductible if you have Medicare and qualify for the sliding fee program.
